A limited number of partial registration scholarships will be awarded to health care profession students, who will be allowed to register for the conference for a fee of $325. Interested students should submit proof of student status and an essay of no more than 250 words, explaining how they would benefit from attending the conference. Applications must be received by February 15, 2018. Send to: Kathie Durban, email@example.com. Please include an email address with all applications. Applicants who do not receive a scholarship will still be allowed to register at the early bird rate after the deadline.
Cancellation and Refund Policy
Cancellations received in writing on or before April 16, 2018, will be refunded less a 15% administrative fee. No refunds will be made after that date. Substitute participants will be accepted.
An updated syllabus is free for attendees online. You may choose to purchase a printed version of the syllabus that includes the speakers' presentation slides received by the print deadline. The recorded presentations are available on-site at the conference for purchase as audio MP3s, or by mail as video DVDs at www.conferencerecording.com.

Guest Meals: $353 per person.
Your guest can accompany you to conference meals. They cannot go to education sessions.
